Answer:	I don't think you need one. What I do is use this item as a base to support my back. Then I stack up my normal pillows on it, so I'm never directly touching the support pillow, itself.I'm not sure if you can get cases for pillows like this, but it might be possible to find a similar item with a removable cover that can be machine-washed. Answer:	I would say yes. You will be sitting at a 15-20 degree angle from upright but you will be virtually upright. Don't be concerned when it arrives severely compressed because it really expands when you follow directions. I am happy with mine. There is no headrest but I don't find that to be a problem.
