The nut on the top of the wheel as it tightens, moves a tapered shaft into this collet, which expands outward to grip the inside of the tube. If you take the top apart, you can see the collet and how the slots will expand outward as you tighten the nut. It works very well. Try to tighten the nut/bolt just a bit before inserting the handle into the wheel shaft. Should tighten and stay tightened this way. At least this has worked for me on both that we have purchased. Also use a screwdriver through the hole on the "tool" provided. This will allow you to tighten it significantly more than without and it should stay tightened.
