The difference between the exercise size and the birthing size is the angle your legs will create when you sit on it with both feet on the ground. The excersize size is smaller because you want to create a right angle or smaller (more acute) in order to get the best workout results and prevent injury. For birthing you want a right angle or higher (more obtuse) you do not want your knees to come below your butt, this will result in a super uncomfortable ordeal.
