Answer:	If you like a close fitting shirt under scrubs, then yes. The fabric is very stretchy. I have a small and I normally wear an extra large. If I wanted a looser fit, I'd probably get a medium, but I don't like it to be bulky.

Answer:	All I can do is copy from the instructions on the Tee itself: 92% viscose/rayon. 8% Elastane/spandex. Wikipedia says that Spandex, Lycra or elastane is a synthetic fiber known for its exceptional elasticity. This means its the same thing just may be named differently (for other countries as it is imported). The 92% is Viscose is both a semi-synthetic fabric, formerly called viscose rayon. I guess they did not want the header to be too long, so they cut out the 8% spandex but showed it under the specifications. I hoped I helped :).

Third, your pack is likely heavier than the one provided. Technically? It could be done. But practically? A lot of jazzing around. An easier alternative would be to buy a second battery pack specifically designed for the shirt and swap out the battery.

Pajamas are usually big for their stated size; however, you want more of a tight fit with thermal underwear as to keep the heat in, and usually warn under regular clothing. So I would say medium. Everything fits differently, but I would say the medium would be a better choice under a large shirt.
