Answer:	I hadn't tried it on carpet but I would think itd work just as fine as long as the carpet isn't super long. You can use the mouse just on the hardwood floor itself or a table top too. You don't have to use the maze board. I just didn't want to get any hair wrapped up in the wheels by using hit on the floor

Answer:	The description says they are made in the USA with GLOBALLY SOURCED ingredients. So, the ingredients could come from anywhere, including China.
Question:	What batteries do these run on
Answer:	Probably, but I will not recommend using them.They gave my Dobie about 15 BB bumps on the top of his head. I went through 3 different bags of expensive dog food and a $65 vet bill thinking he was allergic to his food. Then it dawned on me that the bumps started showing up after I started the vitamins. Once I stopped using them, the bumps started to go away. That was about 6 months ago and he still has 3 remaining, but they are subsiding with time. If I was you, I'd switch your dog to a food that doesn't contain grains - poultry, meat or fish as the first ingredient, followed by brown rice, and veggies. There are a lot of good options available. I hope this helps.
