Answer: With this it is around the widest point of the chest, e.g. Breasts...so if you are 40 around the widest point of breasts get a large..this thing is tight, go exactly by the sizing and don't forget to cough every few hours..after 12hr days of it and not coughing you can cough up blood..otherwise it's awesome binder nice and comfortable

Answer: Definitely American Sizes. Go by the "measure chest" sizing. As a chest 43 / bust 46, I tried the XLarge first. Too tight! Had to trade for a 2X.
